Request Form for Confirmation Cert

European Data Protection law, the General Data Protection Regulation EU 2016/679 (“GDPR”) was introduced on 25 May 2018. This law applies to any organisation, including our parish, that manages and processes people’s personal information.

Therefore, we are obliged to seek identification from anyone requesting a Certificate. If you are seeking a Certificate on somebody else’s behalf, we are required to seek written confirmation that you have authority to collect on their behalf.

Please download the form by clicking the pdf image below. Once completed, these forms together with photographic identification can be handed in or emailed to the sacristy (sacristy@palmerstownparish.com) and we can organise same for you.

Please note the information as provided by you on the Request Forms will be used to draft the Certificate and, once this Certificate has issued to you, this document will be destroyed/shredded.
